DALLAS--(BUSINESS WIRE)--Improved health and more coordinated care for Medicare Advantage members in Dallas/Fort Worth is the goal behind a new population health management partnership between Humana Inc. (NYSE: HUM) and IntegraNet Physician Resource, Inc., an independent physician association (IPA). IntegraNet is an organization that has experienced success in the Houston market in physician population health management and has now expanded into the Dallas/Fort Worth market.
The partnership offers Humana’s Medicare Advantage Health Maintenance Organization (HMO) and Preferred Provider Organization (PPO) membership access to improved population health services provided by a metropolitan-wide network of primary care physicians affiliated with IntegraNet in the Metroplex. Building on IntegraNet’s proven success, Humana members will have access to integrated care teams that offer a higher level of care coordination which has the goal of improving quality of care. These teams, led by physicians, depending on the practice, include an array of providers which may include, nurses, health coaches, medication management, behavioral health and social services resources.
Humana offers Medicare Advantage HMO and PPO plans, a prescription drug plan and Medicare supplement policies to Medicare beneficiaries in Dallas/Fort Worth.

Humana aims to work closely with doctors and hospitals through its Accountable Care Continuum. The new partnership between Humana and IntegraNet includes value-based incentives tied to performance improvement and will bring better patient care coordination, more cost effective, quality care and a world-class patient experience to Humana members in Dallas/Fort Worth. The partnership will offer more personalized care and a broad range of population management tools like predictive analytics and chronic care, disease management and wellness programs.
The Humana/IntegraNet partnership emphasizes quality outcomes with a focus on care coordination and leverages measures as defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) like diabetes care and treatment, breast cancer screenings, colorectal cancer screenings and high-risk medication.
